Private Health Advisory Consultation

Before scheduling your consultation, please take a few minutes to read the information below. This agreement explains the nature of the services provided and helps ensure we have a shared understanding of what to expect.

By scheduling a consultation, you acknowledge that you have read, understood, and agreed to the terms outlined below.

Purpose of the Consultation

Private Health Advisory Consultations are designed to provide educational guidance and wellness information to help you better understand your health, organize information, identify questions to discuss with your healthcare providers, and explore evidence-informed wellness concepts.

Topics may include:

• Health history and lifestyle review

• Nutrition and wellness education

• Functional wellness concepts

• Review of publicly available laboratory information

• Healthcare navigation strategies

• Educational resources

• Sleep, stress, movement, and lifestyle habits

• Goal setting and accountability

• General wellness education

Every consultation is personalized to your individual questions and goals.

Educational Services Only

Salt Honey Sage provides educational consulting services.

Consultations are intended to educate, inform, and support your understanding of health and wellness. They are notintended to diagnose, treat, cure, or prevent any disease or medical condition.

No medical diagnosis, treatment plan, prescription, or medical advice will be provided.

Consultant Credentials

Susan Kobik provides services as the founder of Salt Honey Sage and offers educational wellness consulting within the scope of her education, training, and professional credentials.

She is not acting as your physician, nurse practitioner, physician assistant, registered dietitian, licensed therapist, psychologist, chiropractor, dentist, pharmacist, attorney, accountant, or other licensed healthcare provider during this consultation.

Participation in this consultation does not establish a provider-patient relationship.

Your Responsibility

You remain responsible for your own healthcare decisions.

Any information discussed during your consultation should be considered educational and should not replace advice from your physician or other licensed healthcare professionals.

You are encouraged to consult your healthcare providers before making changes related to medications, supplements, nutrition, exercise, laboratory testing, or medical treatment.

Emergencies

Salt Honey Sage does not provide emergency medical services.

If you experience chest pain, difficulty breathing, stroke symptoms, severe allergic reactions, thoughts of self-harm, or any other medical emergency, call 911 or seek immediate emergency medical care.

Virtual Consultations

Consultations are conducted remotely using Zoom or another secure video platform.

While reasonable precautions are taken to protect your privacy, electronic communication cannot be guaranteed to be completely secure. By participating in a virtual consultation, you acknowledge and accept these inherent risks.

Confidentiality

Information shared during your consultation will be treated respectfully and kept confidential to the extent permitted by law.

Information may only be disclosed when:

  • Required by law
  • Necessary to help prevent imminent harm
  • Authorized by you in writing

Recording Policy

To protect the privacy of both parties, consultations may not be audio recorded, video recorded, photographed, screen recorded, or otherwise reproduced without the prior written consent of both Salt Honey Sage and the client.

Educational Materials

Any worksheets, PDFs, handouts, frameworks, guides, presentations, or other educational materials provided during your consultation remain the intellectual property of Salt Honey Sage.

These materials are provided for your personal use only and may not be copied, reproduced, distributed, published, shared publicly, sold, or incorporated into other products or services without prior written permission.

No Guarantee of Results

Every individual is unique.

Because health outcomes are influenced by many factors beyond the control of Salt Honey Sage, no guarantees or promises regarding specific results can be made.

Scheduling, Payment & Cancellation

Payment is required to reserve your consultation.

Appointments may be canceled or rescheduled up to 24 hours before your scheduled appointment using the link provided in your confirmation email.

Cancellations made with less than 24 hours' notice and missed appointments may be non-refundable.

If Salt Honey Sage must reschedule due to illness, emergency, or unforeseen circumstances, every reasonable effort will be made to offer an alternative appointment.

Governing Law

This Agreement shall be governed by the laws of the Commonwealth of Pennsylvania.

Acknowledgment

By scheduling a Private Health Advisory Consultation, you acknowledge that:

  • You have read and understood this Agreement.
  • You understand these services are educational and informational in nature.
  • You understand that no diagnosis, treatment, or medical advice will be provided.
  • You understand that this consultation does not replace medical care.
  • You remain responsible for your own healthcare decisions.
  • You understand that no specific outcomes can be guaranteed.
  • You agree to the scheduling, payment, and cancellation policies described above.
